Person-centered care.
In recent years, person-centered care has become a movement transforming senior living communities throughout the country. This holistic approach to health care focuses on not only the quality of the care provided to residents, but also ensuring the highest quality of life possible. This is done by allowing residents control over their daily lives, so they may make their own decisions and set their own routines. All levels of staff are involved in the care process, as well as the residents’ families. The goal is to provide a familiar, comfortable, and private environment for residents while focusing on what they can do, rather than what they cannot do.
ACO membership.
Accountable care organizations (ACOs) are groups of doctors, hospitals and other healthcare providers, including senior living communities, who come together voluntarily to give coordinated high-quality care to their Medicare patients. The basic idea is that the providers coordinate care among themselves, and as they hit certain quality metrics they share in the savings if they come in under Medicare cost thresholds. Several Springpoint senior living communities have been accepted as members of their local ACOs based on specific criteria, including quality of care, operations, leadership, and cost-effective care. As an ACO participant, our communities are working collaboratively with others to provide a better level of integrated care, reduce hospital admissions, and improve the overall health and well-being of our healthcare residents. While the landscape of healthcare is changing dramatically, our Springpoint communities continually discover better ways to deliver quality care.
Innovations that ensure high-quality care.
To ensure the highest quality care, Springpoint communities utilize electronic health records (EHR). This means our staff has instant access to the most updated information about our residents’ health at all times. Using EHR also improves the lines of communication among our caregiving team, which provides more effective, person-centered care and improves the satisfaction of our residents and their families.
Surprise inspections.
It’s standard practice for all Life Plan Communities healthcare centers (continuing care retirement communities) to undergo regular state health inspections. However, Springpoint takes these standard inspections a step further by conducting our own surprise inspections, checking in on our communities to ensure we’re always meeting our high standards for senior care.
