Saswati Chakraborty of Village Point Rehabilitation & Healthcare Elected to the Board of the Alzheimer’s Association Greater New Jersey Chapter

Village Point Rehabilitation & Healthcare is proud to announce that Saswati Chakraborty, CSW, MBA, CDP, Case Manager at Village Point, has been elected to the Board of the Alzheimer’s Association Greater New Jersey Chapter. The mission of the Alzheimer’s Association is to lead the way to end Alzheimer’s and all other dementia by accelerating global research, driving risk reduction and early detection, and maximizing quality care and support.

Board Members of the Alzheimer’s Association Greater New Jersey Chapter bring the skills of results-oriented thinking, strategic planning and implementation. The recruitment of Board Members is led by the Chapter Executive Director, and is a year round endeavor. It is a collaborative effort between the Executive Director and current Board Members, who participate in the recruitment of other high level volunteers for the Board and its workgroups.

Newly elected, Ms. Chakraborty has more than 25 years of experience in the healthcare industry and 18 years of experience in marketing business development and admissions processes for several major companies. As Clinical Case Manager at Springpoint’s Monroe Village and Village Point Rehabilitation & Healthcare, she provides coordination of clinical case management across all functions as a liaison between hospitalist providers, acute and post-acute care facilities and other service providers like insurances and ancillary services. She specializes in assessment, treatment planning, linking, advocacy, and monitoring with specialized training in ACOs, bundled payments for care improvement, reducing re-hospitalization initiatives, and managed care organization single case agreements.

Ms. Chakraborty has received specialized training in Alzheimer’s Dementia, and recently completed an intensive six-month certification course in ‘Understanding Dementia’ from Wicking Dementia Center, University of Tasmania. She is also a PAC Dementia Trained Coach by Teepa Snow. Trained by the Alzheimer’s Association in 2011 as a Facilitator of Caregiver Support Groups for Alzheimer’s Dementia, she has been facilitating multiple support groups including one for Early Onset Alzheimer’s.

Ms. Chakraborty earned her Ph.D. in Clinical Psychology and a double master’s degrees in business administration and clinical psychology, respectively. A certified social worker and certified dementia practitioner; she is also a member of the Geriatric Advisory Council.
